Submitting incident reports
Report incidents, near misses, or safeguarding concerns directly from the app.
If something unexpected happens during a visit, carers can submit an incident report directly from the Teiro app. Incident reports are time-stamped, stored securely, and visible to coordinators immediately.
What counts as an incident
- Injury to the customer or carer
- Safeguarding concerns
- Customer distress or behavioural incidents
- Property damage
- Near misses that could have caused harm
How to submit
Open the job and tap "Report Incident". Select the incident type, describe what happened, and upload any supporting photos. Submit and the coordinator is notified instantly.
Severity levels
Mark the incident as Low, Medium, High, or Critical. Critical incidents trigger an immediate alert to all Admin users.
Confidentiality
Incident reports are only visible to users with the appropriate permission level. Carers can see incidents they submitted; they cannot see incidents submitted by others.
