Managing care plans
Create, update, and maintain care plans for each customer within Teiro.
Every customer in Teiro has a care plan record that guides service delivery and supports your organisation's quality documentation requirements.
Creating a care plan
From the customer's record, open the Care Plan tab. You can upload an existing PDF or create a structured plan directly in Teiro using the care plan template.
Care plan fields
A Teiro care plan captures: support goals, daily routines, communication preferences, medical alerts, risk management notes, and emergency contacts.
Review and sign-off
Set a review date for each care plan. When the review is due, the customer's coordinator receives a reminder. After updating, the plan requires digital sign-off before it becomes active.
Version history
Teiro keeps all previous versions of the care plan. You can view any historical version and see exactly what changed between reviews.
Sharing with carers
Relevant care plan information is surfaced automatically on the job screen in the carer app — carers see what they need for that customer without accessing the full plan.
